We have an exciting opportunity for a Nurse Practitioner – House Calls role in a rapidly expanding healthcare organisation delivering accessible, high‑quality medical services across diverse communities. This role involves providing patient‑centred clinical care through home visits, supporting individuals with chronic conditions, mobility limitations, or restricted access to healthcare facilities. The position emphasises holistic assessment, ongoing management, and continuity of care within a collaborative clinical network.

Job Requirements:

  • Master’s degree from an accredited Nurse Practitioner program in Family or Adult‑Geriatric practice.
  • Active Nurse Practitioner license for the state of New York.
  • National board certification through AANP, ANCC, or ONCC.
  • Chinese language skills are an advantage.
  • Experience using EMR/EHR systems is preferred.
  • New graduates are welcome to apply.
  • Ability to work independently while conducting home‑based patient care.
  • Remote and hybrid work options are available.

Responsibilities

Job Responsibilities:

  • Perform comprehensive health assessments during home visits, including physical exams, medication reviews, and evaluation of medical histories.
  • Diagnose conditions and develop personalised treatment plans, prescribing medications when appropriate.
  • Collaborate with physicians, specialists, and care teams to ensure coordinated and continuous patient care.
  • Educate patients and families on disease management, lifestyle changes, medication adherence, and preventive health practices.
  • Maintain accurate electronic health records documenting assessments, treatments, and follow‑up actions.
  • Conduct follow‑up visits to monitor progress and adjust care plans as needed.
  • Respond to urgent medical situations during home visits and provide timely interventions or referrals.
  • Participate in community outreach initiatives to promote available services and support local health education.
